Long Term Care Consultation/MnCHOICES
Long term care consultations are available for all residents of the state of Minnesota to help people identify services they might need or want. A Public Health Nurse or Social Worker help people and their families choose services that reflect their needs and preferences.
The long term care consultation/MnCHOICES is free of charge.
Services Provided
Public Health Staff provides the following:
- Information and education about local long term care options
- A home visit to assess needs and help plan services
- Information about public programs that can help pay for services
- PCA Assessments
- Transition assistance for people who want to return to the community setting after admission to a nursing home/ICF
Public Health Staff will make a recommendation for community services or a nursing facility/ICF when community based services are not feasible. The person makes the final decision about services.
