Long Term Care Consultation
Long Term Care Consultations are available for all residents of the state of Minnesota to help people identify services they might need or want.  A Public Health Nurse or Social Worker help people and their families choose services that reflect their needs and preferences.

The Long Term Care Consultation is free of charge.

Public Health Staff provides:

  • Information and education about local long term care options,
  • A home visit to assess needs and help plan services,
  • Information about public programs that can help pay for services, and
  • Transition assistance for people who want to return to the community setting after admission to a nursing home.

Public Health Staff will make a recommendation for community services or a nursing facility when community based services are not feasible.  The person makes the final decision about services.

Contact the Intake Nurse at 651-385-6100 for more information.
