Home Care
Through the Home Care Program, a nurse provides skilled nursing and coordination of services to ensure that persons with illnesses and/or disabilities can remain in their own home at the maximum level of independence that their health status allows. This can delay or prevent hospitalization or nursing home admission. Home Care services include nursing, home health aides, homemakers, case management, and physical, occupational, and speech therapy.
